1) Improper bulb wattage which creates too much heat - corrected by using cooler LED or CFL bulbs
2) Bulbs are too close to the glass shade - corrected by using smaller bulbs or by increasing distance from shade to bulbs
3) Damage from dropping or tipping over - corrected by placing lamp in more secure location
